Please fill in all of the fields that are applicable. We will send you an email as soon as possible to acknowledge your order, and to request an official purchase order (PO). Once we have received your PO we will invoice you and dispatch your order.

Please note: For non-UK customers we ask that you provide an import licence to prevent delays in Customs. If you do not provide an import licence the Biobank will accept no liability for loss of activity resulting from this. Thank you.
